Start Your Journey with Us
Our Access to Care Specialists will collect basic information about what you’re looking for and what services are covered by your insurance or health plan. We will explain our walk-in option for new clients or you can schedule an intake appointment.

Before Your First Appointment
You will receive a text message with all of our intake forms prior to your initial appointment. You can complete these forms from your smartphone or a desktop.
Or, click here to download the intake forms, print and bring them to your in-person appointment.

Your First Appointment
If this is your first time meeting with a behavioral health clinician, a little anxiety is perfectly normal. Prior to your first session, you will need to complete your paperwork with one of our team members or with the clinical team member you are scheduled to meet with. Then you will meet with a licensed provider for a private conversation about what brought you in for services and develop a treatment plan to help you reach your wellness goals.
This will take about 90 minutes with you to gather information about your needs and to get to know you a little bit so that together you can discuss treatment options available at Recovery Hub.
Please complete all of your paperwork prior to your first appointment, if possible by filling clicking on ‘apply for services’ on our home page.
